Summary: The Care Coordinator will develop service plans and coordinate the care of clients.
Principal Duties and Required Skills:
- Coordinate Admissions
- Make ongoing clinical decisions for assigned clients in collaboration with treatment team and owners
- Coordinates the medical care of assigned clients
- Completes client documentation
Minimum Requirements:
- Fully vaccinated and boosted for Covid-19 (supporting documentation will be requested)
- Master’s Degree in a mental health related field and a minimum of two years of experience, or Bachelor’s Degree in a mental health field with a minimum of five years of mental health related experience
- Experience in a psychiatric rehabilitation or a clinical care setting working with clients who have severe and persistent mental illness
- Possession of a valid driver’s license and satisfactory driving record required. Must have access to a personal vehicle, may be required to transport clients in personal vehicles.
Physical Demands:
- Must be able to move efficiently throughout the day, including stairs, able to stand and/or sit for long periods of time.
- Must be able to lift 10-30 pounds as needed per day.
